My Team forms

To access your forms, you need to open a session on the web site using the log in option of the Intranet menu.

Therfore you will need a username and a password.  If you are a team's manager and you did not received your login informations, please contact Caroline Comeau by email.

Once logged in, you will be able to fill the TEAM BIO form.
